Why do patients request diagnostic tests? What emotions arise for clinicians when patients ask for tests? Why do clinicians find these requests more challenging than those for opioid pain medications? This webinar will provide participants with tools that enable them to have these difficult conversations in short periods of time, which will result in meeting the patient’s agenda while also protecting patient safety.

About the Presenter: 
Auguste Fortin VI, MD, MPH, FACP, FAACH is Director of Psychosocial Curriculum for the Yale Primary Care Internal Medicine Residency Program. He is a Professor of Medicine at Yale University School of Medicine and is Director of Communication Skills Training there. He received his M.D. from Tufts University School of Medicine. He trained in Internal Medicine/Primary Care at Bellevue Hospital/NYU Medical Center and obtained his M.P.H. from The Johns Hopkins University School of Hygiene and Public Health. He was a Fellow in internal medicine at Johns Hopkins University School of Medicine. He is a Fellow of the American Academy on Communication in Healthcare (AACH). Professional interests include doctor-patient communication, spirituality in medicine and the psychosocial aspects of medical care. He enjoys fatherhood, singing, sailing, and flying, and lives in New Haven, CT with his wife Oi, and daughter, Camille.

You, Me and the Computer Makes Three: How to Integrate the Computer in the Exam Room for Optimal Patient Experience - What Learners Should Know - The advent of electronic health records (EHR), coupled with nearly $30 billion in government stimulus to encourage EHR implementation, has prompted a 78 percent adoption rate of EHR and a rapidly increasing number of wired examination rooms, posing a conundrum for clinicians and patients, both of whom are interacting with a “third presence” in the room. Integrating computers in the exam room, while maintaining patient-centeredness, is a challenge at one end of the spectrum and a disaster at the other end.

Emerging from this technological intrusion are effective practices that light the way for clinicians to optimally incorporate computers in routine patient visits. The Cleveland Clinic and the American Academy on Communication in Healthcare developed a communication skills program to improve skills that address enhanced patient experience, improved physician well-being and improved quality of care. In response, one definition that the Institute of Medicine uses to define quality of care is reducing overuse, underuse and misuse. This session will share an approach to engaging physicians in the process of identifying and reducing overuse of services.

Leading the way by communicating for accountability in a culture of safety is designed for those looking to strengthen their skills in communicating for accountability. The three part series begins with an experiential exercise where expectations are not clearly identified and this typically leads to a mishap. We then explore what might have been a safer way to communicate expectation to prevent harm to a patient. In the second session, we focus on using the ARTS dialogue skills (Ask with skilled inquiry, Respond with empathy, then Talk about your perspective in behavioral terms, and Seek collaborative solutions). In the third session, we focus on using a safety algorithm to assess case studies for behaviors (mistake, at risk, or reckless) and determine the appropriate leadership action (consoling and education, coaching, or corrective action). We conclude with the skill of ending a delegation and working on a case each person is currently struggling with in their current environment.
